Cross-sectoral patient handovers

Specialization and centralisation of care is increasing in most healthcare systems. It aims to save resources and increase quality of care. If to succeed the ability to conduct smooth patient handovers is crucial. This includes cross-sectoral handovers between the general practitioner, municipalities, and hospitals. Each handover involves increased risk of mishaps in the patient’s care trajectory, not only due to information and time wasted, but also due to loss of plan, target, values, relations, and trust when patients are handed over.

Our research targets
  • The involvement of patients and relatives in the flow of information and planning of the trajectory.
  • Sharing of relevant information across patients, relatives, and caretakers in different parts of the healthcare system.
  • Improving cross-sectoral personal relation building between caretakers.
  • The development of tracks to fast and smooth handover the right patients to the right caretakers regardless of whether it is from hospital to general practice and municipality care or the other way around.
  • In which situations the patient should come to the specialist and in which situations it should be the other way around – the abilities of virtual meetings.
