Change of Supervisor

The Head of PhD School can appoint new supervisors. This is done at the request of the Head of Department. For the purpose of the request, the accompanying form is used. It must be clear from the form that the Head of Department, the new supervisor and you as a PhD student have accepted the change of supervisor.
